SHG Fee Info:
At SHG, we operate as a private practice, and full payment is required on the day of your appointment. Please feel free to discuss fees during your visit. If you are eligible for subsidies under a care plan, consult your GP about obtaining a Chronic Conditions Management Plan (GPCCMP). This plan allows you to access up to 5 Medicare-subsidized visits for diabetes education. Make sure to email or bring a copy of your care plan to your appointment.
For ATSI patients, you may be eligible for additional support. Speak with your GP about completing an ATSI Health Assessment, which can provide access to up to 10 Medicare-subsidized visits for diabetes education.
Once you have used all your GPCCMP-covered visits, private fee-based appointments are available to continue your care.

Our Commitment to Equitable Access for People Living with Diabetes
At the heart of everything we do is a simple, yet powerful goal: to provide equitable access to all living with diabetes. We believe that no matter who you are, where you live, or the challenges you face, everyone deserves the same opportunity to manage their health effectively.
Diabetes impacts millions of lives, and addressing this requires understanding, compassion, and action. By breaking down barriers to access—whether they’re financial, geographic, or informational—we’re striving to create a future where managing diabetes is easier, fairer, and more inclusive for all. Together, we can make this vision a reality.
